I hereby grant to Mundo Verde Bilingual Public Charter School (Mundo Verde) and their assignees the right to record the image and/or voice and use the artwork and/or written work of my child on videotape, on film, in photographs, in digital media and in any other form of electronic or printing medium and to edit such recording at their discretion.
I understand that my child’s full name, address and biological information WILL NOT be made public. I further grant Mundo Verde and their assignees the right to use my child’s image and/or voice on the internet, in brochures, and in any other medium, and hereby consent to such use.
I hereby release Mundo Verde and their assignees and anyone using my child’s image and/or voice, artwork and/or written work pursuant to this release from any and all claims, damages, liabilities, costs, and expenses which I or my child now have or may hereafter have by reason of any use thereof.
I understand that the provisions of this release are legally binding.

Otorgo permiso a la Escuela Pública Charter Bilingüe Mundo Verde (Mundo Verde) y sus representantes, para grabar la imagen y/o la voz y usar material gráfico y/o material escrito de mi hijo/a en cinta de video, en películas, en fotografías, en medios de comunicación digital, y en cualquier otra forma de medios electrónicos o de impresión, y a editar dichas grabaciones a su discreción.
Entiendo que NO SE PUBLICARÁ el nombre completo de mi hijo/a, ni su domicilio, ni su información personal. También le otorgo a Mundo Verde y sus representantes, el derecho a usar la imagen y/o la voz de mi hijo/a en internet, en folletos, y en cualquier otro medio; y doy mi consentimiento para dicho uso.
Eximo a Mundo Verde, sus representantes y cualquier persona que use la imagen y/o la voz de mi hijo/a, material gráfico y/o material escrito, conforme a esta exención de cualquier y de todos los reclamos, daños, responsabilidades, costos, y gastos en los que mi hijo/a o yo incurramos actualmente o podamos incurrir debido a cualquier uso del mismo.
Entiendo que las disposiciones de esta exención son legalmente vinculantes.
